Unihealth Source Program
Service Options Using Resources in Community Environments (SOURCE) is a program designed to provide elderly, disabled and low-income persons receiving Medicaid SSI with a wide range of long-term health services, either in their home or the community.
Program requirements are:
- 65 years old or older and/or disabled
- Receiving Medicaid and SSI
- Medical conditions such as diabetes, high blood pressure, lung disease, arthritis, Alzheimer’s disease, dementia, heart disease, or stroke that require monitoring and regular doctor’s care
Affordable Medical Resources, Inc. is licensed by Unihealth Source to provide Personal Support Services. Affordable Medical Resources has been serving the community since 2003 and is currently serving 150 active clients. At Affordable Medical Resources, Inc. we work with our clients or their families to develop personally tailored services to best serve your needs. Our goal is to provide services that maintain or increase the functioning capacity of the clients being served and focus on the relationship between the clients and their needs.
